As a member of Aqua Medical Spa, we hope you will enjoy the many benefits that are part of your membership. These benefits include the following:

The initial membership period shall be for a period of six months (the “Initial Period”). A member shall not be entitled to terminate or suspend his/her membership during the Initial Period. If the contract is terminated before the expiration date, there is an early termination fee of $100.00 that will be charged to the card on file. Any contract that has been terminated prior to the expiration date will have a waiting period of 1 year before being able to reinstate.
After the Initial Period, the membership shall automatically set up to renew for a six-month period. Either party may cancel the membership at any time by giving a 30 day written notice to the other party.
This Agreement is personal to the member(s) and may not be assigned, transferred, or otherwise disposed of by the member(s).
You will be required to sign our Membership Agreement on your first visit.
Cancellation must be presented in writing within 15-day notice after the Commencement date.
You may cancel with a written notice to your Aqua Medical Spa location.
Aqua memberships are not to be used in combination with any other office promotions or discounts.
Aqua Medical Spa reserves the right to review subscriptions periodically. Members will be given at least a 30 days’ notice in writing of any changes.
Aqua Medical Spa reserves the right at any time to cancel or suspend the membership.
A complimentary skin consultation with our aesthetician is required prior to becoming a member.
